The North Haven Library is NO LONGER a location for VITA tax preparation services.  VITA is offering tax prep at the Wallingford Library and the Edward Smith Library in Northford.  Please contact them directly for more information. There may be additional local locations for VITA tax prep, please check the IRS website.

Federal Tax Forms

THE CURRENT YEAR TAX FORMS HAVE NOT BEEN SENT OUT BY THE IRS YET.  FOR YOUR CONVENIENCE, THE LIBRARY HAS PRINTED SOME COPIES. EACH PERSON IS LIMITED TO TWO PRINTED COPIES UNTIL WE RECEIVE THE TAX FORMS FROM THE IRS.

The IRS provides downloadable tax instructions, helpful publications and current forms.  The IRS also sends the North Haven Memorial Library paper copies of tax forms and instruction booklets which are located in the library by the computers.

The State of Connecitut has printable tax forms,  fillable tax forms and links to file electronically.

What you need to get started:

Personal Information

  • A copy of last year's tax return

  • Verify your identity by using your 2016 Adjusted Gross Income or your 2016 personal identification number if you created one

  • Don't have your AGI or PIN? If you do not have your 2016 tax return, you may use Get Transcript Online or Get Transcript by Mail to obtain your prior-year AGI. you also may call 1-800-908-9946 for mail order. Please allow 5 to 10 calendar days for mail delivery. Get Transcript Online users must first pass Secure Access authentication.   • Valid Social Security numbers for yourself and your spouse, if applicable

  • Valid Social Security numbers for your dependents, if applicable

  • Income and Receipts
  • Social Security benefits

  • Unemployment Compensation

  • All receipts pertaining to your small business, if applicable

  • Income receipts from rental, real estate, royalties, partnerships, S corporation, trusts

  • Other income
  • W-2s, showing your annual wages from all of your employers

  • Forms 1099-INT, showing interest paid to you throughout the year

  • Form 1099-G, showing any refund, credit or offset of state and local taxes

  • Forms 1099-DIV and Forms 1099-R, showing dividends and distributions from retirement and other plans paid to you during the year

​ACA Filers

  • Form 1095A, Health Insurance Marketplace Statement

  • Form 8962, Premium Tax Credit

  • Form 8965, Health Coverage Exemption and ECN 

File Electronically

  • Verify your identity by using your 2016 Adjusted Gross Income. If you created a 2016 personal identification number, that will work too

  • Don't have your AGI? Call the IRS toll-free line at 1-866-704-7388

Contact Information

  • To get a notification from the Free File software company that your return was accepted by IRS, you need a valid email address

Income above $64,000: Free File Fillable Forms 
  • Free Fillable, electronic versions of the paper forms

  • Must know how to do your taxes yourself

  • Does math; offers only basic guidance

  • State tax prep is not available
